The method (3000) comprises generating an n-dimensional dataset from a plurality of raw health data, wherein the n-dimensional dataset comprises a plurality of data points related to health of the entity, and wherein the n-dimensional dataset comprises high dimensional matrix of the plurality of data points. The method (3000) further comprises reducing dimensionality of the n-dimensional dataset to create a lower-dimensional dataset. The method (3000) further comprises generating a topological representation of the lower-dimensional dataset to create a personalized health portrait of the entity and analyzing the personalized health portrait to predict health state of the entity based on the analysis."
