Abstract: An artificial intelligence-based system is provided for monitoring multiple physiological parameters and supporting clinical decision making. The system includes an input layer configured to receive real time physiological telemetry from IoT medical sensors and bedside patient monitors and to receive scanned or handwritten clinical documents. A processing layer extracts time references from the telemetry and documents and generates a unified patient timeline. A supervisor inference engine comprising a fine-tuned large language model analyzes the unified patient timeline to generate a provisional diagnosis, a differential rule out, a treatment recommendation or a risk score. An output layer presents the generated information on a clinical dashboard and a remote telemedicine application and interfaces with a hospital information system for storing and retrieving patient data. The system enables contextual interpretation of physiological signals together with clinical records to improve diagnostic accuracy, treatment planning and remote specialist consultation. Fig. 1
