Abstract: The present invention describes a computerized system and method to enhance the discharge process of patients. This computerized system uses the mobile computer device combined with different health information systems to achieve coordinated activities among the clinical teams in real-time to check the discharge readiness of the patient, maintain the safety of the patient, and ensure safe discharge procedure at the point-of-care. The system includes Hospital Data Integration Layer (HDIL) to gather, validate, normalize, synchronize, and disseminate the clinical, operational, administrative, and workflow data related to the patient in different healthcare systems. The synchronized data is analyzed using several modules which include Adaptive Clinical Readiness Synchronization Engine (ACRSE), Operational Clearance Engine (OCE), Patient Preparedness Engine (PPE), and Risk-Weighted Patient Safety Goal Scoring Engine (RW-PSGSE). An Intelligent Adaptive Discharge Orchestration Engine (IADOE) will coordinate the discharge process through coordination of multidisciplinary activities, recognition of workflow interdependencies, task prioritization, and constant monitoring of a Dynamic Discharge Readiness Score (DDRS). A Dynamic Safety Lock (DSL) system will ensure that discharge will occur only when all the clinical, operational, administrative, and safety requirements are satisfied. Additionally, an Adaptive Predictive Workflow Engine (APWE) identifies potential delays and bottlenecks in the discharge process, while a Multidisciplinary Workflow Synchronization Engine (MWSE) coordinates simultaneous discharge activities. The mobile computer device supports secure bedside verification, documentation, electronic acknowledgments, and synchronized execution of discharge steps. After discharge approval, the Automated Discharge Documentation Generator (ADDG) generates customized discharge documents, while the Post- Discharge Continuity Module (PDCM) launches after-discharge care programs. The system offers an event- based, dynamically updating platform for coordinated, secure, and standardized patient discharges in interconnected health care settings.
