Abstract: The present invention relates to a system (100) and a method (300) for providing intraoperative optical guidance. The method (300) comprises acquiring, by a processor (201), multispectral optical signals emitted from a tissue surface upon activation of a multispectral fluorescent formulation applied thereto, separating the multispectral optical signals into a plurality of spectral channels, and computing a margin confidence score, a leak risk index, and a perfusion integrity score for each pixel of the multispectral optical signals from the plurality of spectral channels. The processor (201) computes a composite surgical safety score from a weighted sum of the margin confidence score, the leak risk index, and an inverse of the perfusion integrity score, determines a risk category therefrom, and generates a visual overlay assigning a colour corresponding to the risk category of each pixel and displays the visual overlay via a user interface (207).
