Abstract: In response to the growing need for continuity, accuracy, and efficiency in home healthcare services, this project focuses on developing an AI-Powered CareShift Handover and Report Generation System. The application is designed to support professional caregivers such as nurses and attendants by providing a structured and secure platform to record, view, and transfer patient care information across shifts. The system enables caregivers to manage multiple patients, document shift-wise observations such as vitals, medications, and care notes, and seamlessly view previous shift summaries added by other caregivers. An AI-driven module generates concise and meaningful shift reports from caregiver inputs, reducing manual effort and minimizing communication gaps. The application operates under role-based access control, ensuring that only authorized healthcare professionals can access patient data. By centralizing patient records and enabling real-time handover between caregivers, the system promotes ethical usage, accountability, and improved quality of home-based patient care. This solution enhances care continuity, reduces errors during shift transitions, and supports efficient decision-making, ultimately improving patient outcomes in home healthcare environments.
