The system is implemented as a progressive web application (PWA) built on React.js, Node.js, Express.js, and MongoDB, supporting online and offline operational modes with automatic data synchronization. A QR-based digital health card module provides each patient with a portable, verifiable, offline-accessible health identity encoding encrypted medical records, active prescriptions, and allergy information. An ASHA-worker mediated onboarding interface with regional-language voice guidance enables community health workers to register patients, capture vitals, and schedule teleconsultations without requiring patient smartphone ownership or digital literacy. Low-bandwidth WebRTCbased video consultations with adaptive codec selection and audio-only fallback ensure reliable doctor-patient communication under variable network conditions. An AI-assisted symptom assessment module utilizing a client-side TensorFlow.js multilayer perceptron classifies patient symptoms into severity tiers and generates clinical summaries to guide triage decisions. Cryptographically signed digital prescriptions are relayed to integrated pharmacies with SMS fallback through the Twilio API, and a constraint-based scheduling algorithm prioritizes consultations by severity score and geographic clustering. The system is deployable on AWS cloud infrastructure or as an edge server at district health centres, ensuring continuity of healthcare delivery even in the most connectivity-deprived rural environments.
