Abstract: Disclosed are autologous, transplantable living human skin constructs comprising (i) an epidermal compartment including autologous epithelial cells arranged to form a stratified epidermis, (ii) a dermal compartment including autologous stromal cells disposed within an extracellular matrix environment, and (iii) a basement membrane interface positioned between the epidermal compartment and the dermal compartment. The constructs are configured for transplantation to a human recipient site and for integration with host tissue while resisting long-term failure modes including fibrosis, contraction, mechanical delamination, chronic inflammation, and abnormal remodeling. In some embodiments, the constructs further comprise one or more appendageal integration modules selected from hair follicle niche modules, arrector pili muscle anchoring modules, autonomic nerve capture and stabilization modules, eccrine sweat gland field modules, duct patency stabilization modules, vascular integration compatibility modules, and sensory interface maturation modules. In some embodiments, the construct further comprises a temporary acellular mechanical buffer layer configured to dissipate shear and strain during early engraftment. The invention enables modular, scalable, regulator-compatible living skin products with improved long-term structural stability and physiological equivalence to native human skin.
