Abstract: The present invention relates to a blockchain-based secure Electronic Health Record management system for trusted storage, access, sharing, verification, and auditing of patient medical records. The system comprises a patient identity module, healthcare provider identity module, record ingestion module, encryption and key management module, off-chain medical data vault, permissioned blockchain ledger, smart consent execution module, access control gateway, record integrity verification module, emergency access module, and audit monitoring module. Medical records are encrypted and stored in an off-chain vault, while cryptographic hashes, encrypted pointers, consent parameters, and access audit events are recorded on a permissioned blockchain. The system enables patient-controlled consent, provider authentication, tamper-evident record verification, consent revocation, emergency access governance, and immutable audit logging. The invention prevents unauthorized alteration, improves healthcare interoperability, reduces centralized data vulnerability, and enables secure sharing of medical records among authorized healthcare stakeholders without exposing complete medical data on the blockchain.
