Abstract: ABSTRACT CONNECTED REMOTE PATIENT MONITORING SYSTEM FOR POST-HOSPITALIZATION CARE The present invention discloses an AI powered patient monitoring (PM) system for individuals recently discharged from medical facilities. The system comprising a mobile device having an app, AI engine, connected IoT devices, and a centralized nurse command center for post-hospital care. It integrates a mobile device having application, Bluetooth-connected medical devices, clinician dashboards, and a 24x7 command center to deliver real-time monitoring, symptom tracking, and care plan adherence.
