Abstract: Healthcare is changing fast, thanks to new technology. But keeping an eye on patients once they leave the hospital is still hard. People recovering at home, older adults, or those living far from clinics often don’t get checked regularly. That can mean late treatment and worse health problems. Right now, healthcare usually waits for you to report a problem. It’s not set up to watch over you all the time to prevent issues. That’s why we need a simple live system that connects hospital-level care to everyday health at home. This project introduces a smart health monitoring system built with everyday tech. It keeps track of key health signs like heart rate, blood oxygen, body temperature, and even room conditions like heat and humidity. At the heart of the system is a small computer chip (ESP32) that gathers all the data and sends it safely online. A GPS feature also tracks location in real time, which helps speed up emergency help if needed. All the information is shown on an easy-to-view online dashboard, so doctors or family can check in from anywhere. If something doesn’t look right say, a heart rate goes too high the system sends an alert right away. This gives caregivers a chance to step in early. Over time, the data can also help spot trends and support better long-term care choices. We built and tested a working model to show this idea is practical and affordable. It’s not meant to replace professional medical devices, but it proves how simple tech can help keep people safer at home. In short, this kind of system makes healthcare more proactive not just reactive. By watching health continuously, sending timely alerts, and giving a full picture of a patient’s condition, it can help prevent emergencies, ease pressure on hospitals, and support healthier day-to-day living outside clinical settings.
