Abstract: A low molecular weight protein fraction not exceeding fifty kilodaltons isolated from Pheretima posthuma is disclosed for treatment and prevention of Non-Alcoholic Fatty Liver Disease. The fraction exerts multiple hepatoprotective effects including suppression of TNF-alpha and IL-six, reduction of oxidative stress and lipid peroxidation, restoration of antioxidant enzymes, improvement of hepatocyte viability and normalization of liver disease markers. Pharmaceutical compositions contain this fraction prepared through homogenization and ultrafiltration with fifty kilodalton cut-off. A preferred embodiment comprises sterile aqueous injectable formulation with suitable buffers, stabilizers and surfactants. Illustrative in vivo examples in high-fat diet models show treatment reduces ALT from one hundred twelve units per liter to forty-eight, reduces malondialdehyde from three point eight to one point six nanomoles per milligram protein, restores superoxide dismutase and other antioxidant enzymes, decreases TNF-alpha and IL-six, reduces steatosis and inflammation, and improves NAFLD Activity Score from six to two
