Abstract: ABSTRACT Method and System for Image-Based Multimodal Medical Records with Coordinate-Mapped Workflow-Aware Processing The present invention relates to a computer-implemented system and method for workflow-aware processing of clinical input data and generation of electronic medical records. The system receives clinical input from one or more modalities and determines a current clinical workflow state. The clinical entities are extracted using context-dependent algorithms, assigned confidence scores based on contextual relevance and medical ontology matching, and validated in real time using workflow-specific rules. Based on the workflow state, a documentation template is selected and validated data is mapped into predefined fields. The present invention further provides an image-based storage architecture wherein the electronic medical record is generated and stored as an image file with coordinate-mapped field positioning, enabling rendering of handwriting, voice-transcribed, and digital inputs at specific pixel locations. The structured data may be extracted from the image file when required for interoperability, thereby enabling efficient, accurate, and visually faithful clinical documentation.
