Abstract: The present invention discloses a system for patient-centric electronic health record (EHR) management. The system includes a user interaction layer (302) configured to receive requests for accessing, sharing, or managing electronic health records, and a security and access control layer (304) configured to perform identity verification and authentication using cryptographic credentials and to control access based on predefined authorization rules. A consent and application service layer (306) is configured to evaluate and enforce patient-defined consent policies. A data storage layer (308) stores electronic health records in encrypted form in an off-chain storage system and generates corresponding cryptographic hash values. A blockchain and trust layer (310) records the hash values and access transactions in a distributed ledger to enable data integrity verification and immutable logging. A compliance and audit layer (312) maintains verifiable audit records of access events, thereby ensuring secure and tamper-resistant management of electronic health records. Fig. 1
