Abstract: PREDICTIVE ANALYTICS FOR PATIENT RISK: REAL-TIME IDENTIFICATION AND FLAGGING OF HIGH-RISK PATIENTS Abstract of the Invention The present invention relates to a computer-implemented system and method for real-time prediction and identification of high-risk patients using multimodal healthcare data. The system collects structured and unstructured clinical data, including electronic health records, laboratory reports, clinical notes, and wearable sensor inputs, and processes them using a Time-Aware Transformer-based model capable of handling irregularly sampled time-series data. A multimodal fusion engine integrates heterogeneous data sources to generate a unified patient representation, which is used to compute a dynamic risk score updated in real time. The system further includes an alerting module for identifying high-risk patients and notifying healthcare providers for timely intervention. Additionally, explainable AI techniques and federated learning ensure interpretability and privacy- preserving collaborative training. The invention enables early detection of patient deterioration, improves clinical decision-making, and enhances patient outcomes through continuous, scalable, and adaptive predictive analytics.
