The patient risk score is updated upon receipt of new clinical data from heterogeneous sources in real time by a dynamic risk recalibration unit (108). The patient is assigned to a medical department based on semantic similarity between patient data and predefined department descriptors by a department mapping unit (110). A digital patient record including the assigned risk category and department is automatically created and updated by an electronic health record (EHR/EMR) automation unit (114).
