Abstract: A system and method are disclosed for physiology-gated analysis of invasive arterial pressure waveforms for vasoplegia risk estimation and hemodynamic reserve monitoring. The system receives arterial waveform data, evaluates signal quality, determines a waveform-derived vascular-floor parameter, and computes one or more mean-arterial-pressure-relative perfusion reserve parameters. The vascular-floor parameter may comprise an effective critical closing pressure surrogate, an asymptotic pressure surrogate, or another pressure-floor estimate determined by any suitable model. The system may compute one or more pillar parameters including a reserve ratio, an absolute reserve gap, vascular-floor or reserve-gap dynamics, systolic upstroke compensation features, and confidence information. A confidence engine uses waveform quality, model-fit quality, continuity, baseline reliability, persistence, and physiological plausibility to generate a hemodynamic reserve state, trajectory, risk indication, reason-coded output, or insufficient-data indication. The system may be integrated with existing patient monitoring platforms.
